You may find it convenient to use our Free Email Service when...

1. When you need to know, before you make an appointment, if we can help you with a certain problem, use this form to find out.   

2. When you saw the doctor in the office within the last seven days and

• need to ask a question you forgot during your last visit. 
• want to explain something you think that I may have misunderstood during the visit.

3. When you need a refill. These emails and refills incur no charge if all these conditions are met:

• We prescribed this at an office visit within the last 12 months and
• the medication was intended for long term use and
• the medication is not potentially habituating (not federally controlled) and
• you are up-to-date on any lab or physical examination required to monitor this medication.

If any of these conditions does not apply, and you are a patient of this office, hit your back button and use this form, or telephone our nurse for your refill.
